23-24 LMS Sport Physical Forms
Physicals completed after May 1st, 2023 will be valid for summer & the 2023-2024 school year. All of the above forms are required to be filled out, signed by athlete & parent/guardian, and returned to LMS office to be eligible to participate in any sport.
