Louisburg High: Enrollment

2009-10 LHS Enrollment

Forms, Guidelines

Payments can be made through two options:

    1. check, or
    2. online through Louisburg RevTrak Online (LIVE July 10)
      (need to copy extra receipt to accompany enrollment form)

Paperwork consisting of all forms can be returned through one of three options:

    1. by mail to:     Louisburg High School          
                           P.O. Box 399  
                           Louisburg, KS 66053

        NOTE:  Post Office will not deliver if using 202 Aquatic Drive as address.

    1. to enrollment drop box in front of high school building during the month of July.
    1. in person on August 5, 2009 from 8:00 a.m. - 3:00 p.m. for Schedule Changes
      • Seniors    8:00-9:30 a.m.
      • Juniors     9:30-11:00 a.m.
      • Sophomores   12:00-1:30 p.m.
      • Freshmen          1:30-3:00 p.m.

Parent Letter - instructions

FORMS to be completed and returned to school:
1. 2009-2010 LHS Enrollment Form
(attach a copy of your RevTrak receipt to insure credit if online payment)

LHS Meal FEE Sheet
3. Acceptable Use Policy & AGREEMENT
4, Parking Registration
5. LHS is going GREEN
6. Drug & Alcohol Agreement
